Our Approach to Primary Care : We focus on identifying and treating the root causes of disease. We investigate how and why illness has occurred in your specific case and work with you to restore health by addressing the root causes of your medical issues. To achieve your health goals we will first evaluate and manage your current medical conditions and at the same time create a plan for you to reach and maintain your ultimate health. We use many diagnostic and treatment tools tailored to your specific needs, understanding that no two humans are the same, and each of us needs individualized health care.

Services we offer :
Primary Care covered by insurance
Direct Primary Care (DPC) : DPC is primary care for a nominal monthly subscription fee. DPC is designed for patients who do not have medical insurance.
Medical Aesthetics : Offered by board certified, fellowship trained physicians and Taeko aesthetics.
Disclaimer : We do not prescribe narcotic pain medications, benzodiazepines, stimulants.
It is our belief that chronic pain patients are best served by certified pain medicine specialists in specialized pain management clinics hence we do not prescribe narcotics for chronic pain.
Privacy Policy
Information about your treatment and care, including payment for care, is protected by two federal laws: The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and the Confidentiality Law.
Under these laws CH Medical may not say to a person outside of CH Medical that you attend CH Medical, nor may CH Medical disclose any information identifying you as an alcohol or drug abuser.
Generally, you must sign a written consent before CH Medical can share information for treatment purposes or for health care operations. However, federal law permits CH Medical to disclose information in the following circumstances without your written permission:
To CH Medical staff for the purposes of providing treatment and maintaining the clinical record.
To a business associate (e.g. Clinical laboratories, pharmacy, record storage services, billing services).
For audit or evaluations (e.g. State licensing review, accreditation, CH Medical data reporting as required by the State and/or Federal government)
To report a crime committed on CH Medical’s premises or against CH Medical personnel.
To medical personnel in a medical/psychiatric emergency.
To appropriate authorities to report suspected child abuse or neglect.
To report certain infectious illnesses as required by state law.
As allowed by a court order.
Before CH Medical can disclose any information about your health , it must first obtain your specific written consent allowing it to make the disclosure. We will provide a HIPAA consent form to you before your first visit. Any such written consent may be revoked by you in writing. (NOTE: Revoking a consent to disclose information to a court, probation department, parole office, etc. may violate an agreement that you have with that organization. Such a violation may result in legal consequences for you.)
